Access to Medical Records

You are entitled to access most of the records the Practice holds on you (There are a few exceptions). The Practice makes a statutory charge for this service, as directed by the Data Protection Agency. You can request a Fees and Charges leaflet from reception for more information.

Please contact the Practice Manager for details and to arrange an appointment to view records.

If a Doctor writes a medical report for an outside Agency i.e Insurance Company, you are entitled to view the report before it is sent. Please note there is a time limit. Ask Practice Manager for details.

When a Doctor writes a letter about you, you are entitled to a copy; discuss with the Doctor or Practice Manager.

Please note: the Doctor will not divulge any information the Practice holds on you, to any other Agency, Organisation or Family, without your signed consent. You may discuss arrangements with the Practice Manager.

You can now view your basic medical summary online through Systmonline. Once registered you will have access to your list of recent medications, any adverse reactions and allergies you may have. Please click here for further information.

Access to Patient Information

Everyone working in the NHS has a legal duty to maintain the highest level of confidentiality about patient information. Your medical history is/will be recorded on the practice computer system and in paper records, these records are accessed by authorised users only. Data Protection Act

We are registered under the Data Protection Act and have robust systems in place to protect your confidentiality. Personal health information is used to monitor the practices audit activities. Occasionally anonymised health information is sent to the CCG to monitor quality standards and for post-payment verification purposes. Anonymised patients' data may also be used for research that is in the best interests of patients and the NHS as a whole.

Great North Care Record

The Great North Care Record is a new electronic system enabling NHS Health Professionals that are providing you with treatment to view a summary of the records held with you're GP.

NHS Summary Care Records

As part of a mandatory, national programme each GP Practice will have to make a summary care record for each patient (unless the patient has already opted out).

Please read the information regarding this.

Transferring Your Electronic Health Record

Your GP practice holds copies of your patient health record electronically and in paper format. Both contain the healthcare information about you that your GP needs including your medical history, medications, allergies, immunisations and vaccinations.

If you have previously registered with a different GP in England, upon registering at this practice your electronic health record will, where possible, be transferred automatically from your previous practice through the use of an NHS system called GP2GP.
