Registering with Us


Lintonville Medical Group is a Non-Limited Partnership contracted to the National Health Service via the North of Tyne, Northumberland Care Trust. You will be registered with Lintonville Medical Group and not a specific General Practitioner, however you will be allocated a named GP (see below). 

All GP’s have access to your electronic medical record to ensure continuity of care.

The NHS is promoting a Summary Care Record to help you in the event of an emergency situation.

Accountable / Named GP for All Patients

All patients registered at the Lintonville Medical Group have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

New patients are allocated a Doctor on Registration.

If you are unsure, you can also contact reception to confirm. 

If you change your name and address or telephone number please inform us as soon as possible. If you move out of the practice area, you will be required to re-register with a more local practice.

How to Register as a Patient

If you live in our practice catchment area (see above) you can join our list. If you are registering your new baby please next section.

To register please visit the surgery with the following information:

  • Your Medical Card or, if you do not have a Medical Card please complete a GMS 1 form
  • To ensure you are provided with appropriate medical support we would also ask that you complete our new patient questionnaire.
  • 1 x Photo identification (eg Passport or Driving Licence)
  • 1 x Utility bill or bank statement sent to your current address, which you have received within the last three months.

How to Register your New Baby

If you are a patient and would like to register your baby please bring their Birth Certificate as soon as possible after leaving hospital to allow a medical card to be provided.

Visiting Patients (Temporary Residents)

UK or EU Residents

If you are a UK or an EU Resident visiting the area you can register as a temporary resident and receive medical care. You will need to provide appropriate documentation including Passport, Driving Licence, Birth Certificate or Medical Card to support your request to access NHS services. You will need to complete our Temporary Resident Registration form which is available from reception, or can be downloaded from the 'Registration Forms' section on this page.

Non UK or EU Residents

Non UK or EU Residents are not entitled to free NHS Services at Lintonville Medical Group. Further information is available on the 'NHS charges for people from abroad' page of the Citizens Advice website.

You can access free of charge NHS Choices and NHS 111 for advice.

Our priority is to registered patients and temporary resident from with the EU. If an appointment is available this will be offered to non-EU temporary residents, however a charge will be made.
